Online forms: Computer Screen Assessment

All council staff that are considered a 'user' of Display Screen Equipment (they work continuously for an hour or more at a computer screen) should complete the online self assessment form.

Staff should book a Display Screen Equipment assessment with the Corporate Health and Safety Officer if they:

• Have regular aches or pains to your back, neck, shoulders, wrists, hands, legs etc
• Have a disability that needs special adjustments or precise set up posture to make you comfortable at your work station
• Are pregnant
• Are struggling to get their set up comfortable on their own and would like extra advice
