Changes to your licence
If you have a problem downloading the application form, right click the download link and choose “Save link as” and save to your computer. Then go to the file, right click and choose “Open with Adobe”.
Minor/Major variations
Depending on the change you wish to make it will be classed as either a minor variation or major variation.
Designated Premises Supervisor (DPS) variation
To change the DPS at the premises you must submit both the variation application form and the consent form below:
- DPS variation application form - to be completed by premises licence holder
- DPS consent form - to be completed by the prospective DPS
The fee is £23.
Unless you submit the application online or by email, you must send a copy of the DPS variation application and consent form to the Police:

Transfer of licence
To change the premises licence holder i.e. transfer the licence, you must submit both the transfer application and the consent form:
- Application to transfer premises licence – to be completed by the prospective premises licence holder
- Consent to transfer premises licence – to be completed by the existing premises licence holder
The fee is £23.
The prospective licence holder must demonstrate their entitlement to work in the UK by providing evidence from the list of acceptable documents on the application form.
Unless you submit the application online or by email, you must send a copy of the transfer application, consent form and entitlement to work proof to both the Police and Home Office:

Change of name/registered address of licence holder
Please notify us in writing if the name or registered address of the premises licence holder changes or if you wish to change the name of the premises. There is a £10.50 administrative fee applicable.
